Confused about Shutter Island?

I understood the movie mostly, I think. But, why then did the Dr. lady in the cave, Rachel, tell Teddy(Leonardo) that the prison wanted to mess with peoples brains and use an ICEPICK on them through the eye? Then, at the end, you saw the Icepick wrapped in a Linen towel in the workers or guards hands when they took Teddie (Leonardo). Why would that happen if the event of him seeing the Dr. in the cave never really happened? Maybe it did. Maybe they just were just giving him drugs to make him more dilussional since he did kill his wife because she killed his kids. Does anyone know? Explain this? The icepick? This is what I don't understand?

I think that he wasn't crazy and that they tricked him into staying there because the lady in the cave said you know they can't let you leave.
In the opeining scene he couldn't find his cigarettes on the boat and his partner was like here you can have some of mine. So from the start he was drugged with hallucinagents.
They really brought him there because he was researching their facility and finding out about
there experiments.
That's my point of view!
